Thank you for choosing Atlantis School of Swim. We are now pleased to offer on-line registration which can be accomplished in 2 easy steps:
1. Submit Form: Please fill out the form below with all relevant information. 2. Payment: Once your spot has been reserved, you will receive an Email with a link to a PayPal invoice or an Email with further details regarding cheque/EMT payment, depending on the method of payment selected in your registration. If using PayPal, please follow the link to pay for classes online. Payment is due within 3 business days of receiving notification. Spots are not held after 3 business days. Once payment has been received in full and your registration has been processed, your spot is guaranteed and you will receive confirmation with the details of your lesson schedule.

Permission to use media:
I hereby give permission to Atlantis School of Swim Inc. to use photographs, social media, video tape, sound record and/or film and any other media representation of myself and my family members for promotion and/or advertising. I release Atlantis School of Swim Inc., its officers, directors, volunteers and the staff from any and all responsibility/liability that may arise as a result of the use of such photos/media.

I agree and understand that there are certain risks or hazards inherent in the participation of swimming lessons. The participant hereby agrees to participate in the Learn to Swim programs provided by Atlantis School of Swim Inc. and hereby agrees to forever release, discharge and to indemnify and save harmless Atlantis School of Swim Inc., its facilities, its coaches, officers, directors, agents and employees against any liability resulting from any injury that may occur to the participant while participating in the Learn to Swim program. The participant also agrees to indemnify Atlantis School of Swim Inc. for any damages incurred arising from any claims, demand, action or cause of action by the participant. The participant authorizes any representative of the Atlantis School of Swim Inc. to have the participant treated in any medical emergency during their participation in the Learn to Swim program. Further, the participant and or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant. I have noted on this form any medical/health problems the staff should be aware. Atlantis School of Swim Inc. is not responsible for lost or stolen items.
Cancellation and Refunds - Once you have registered and committed to a spot there will be no refunds or credits. Please be aware that Atlantis School of Swim Inc. incurs costs as soon as you register for our programs. We pay registration, insurance for each swimmer, sign contracts with coaching staff based on numbers registered and contract and pay rent in advance of your session for pool time with our facilities. Therefore, we regretfully are unable to offer refunds or credits for our swimming programs.
Change requests to switch days/times within the same session may be accommodated, depending on availability and less a $25.00 change fee.
Make-up Classes - Due to limited space and pool time we are unable to accommodate any requests for make-up lessons. Missed lessons as a result of client absence DO NOT qualify for make-up classes. In the event of lesson cancellation by Atlantis School of Swim due to unforeseen pool/facility closures, we will do our best to re-schedule your lesson for another day/time or alternate location should an option be available. In the event of lesson cancellation due to inclement weather at our indoor facilities, make-up lessons will only be offered in the event of a facility closure. Should the facility remain open and a closure occur to ensure the safety of clients and staff, then a make-up lesson will not be offered.
